Tiotropium Prevents Exacerbations in Some Patients with Severe Asthma.

In patients with poorly controlled asthma, this agent might be a reasonable option.

Some patients with asthma continue to experience poor lung function despite receiving long-acting β-agonists (LABAs) in combination with inhaled corticosteroids (ICS). Tiotropium is a long-acting anticholinergic (approved for chronic obstructive pulmonary disease) that has shown promise in short-term trials for treating asthmatic patients (JW Gen Med Jun 14 2011).

In two replicate, industry-sponsored, randomized trials, 912 adult patients received tiotropium or placebo. All patients experienced at least one severe exacerbation in the previous year and had persistent airflow limitation (mean post-bronchodilator forced expiratory volume in 1 second [FEV1], 62% of predicted), despite therapy with high-dose ICS and LABAs, which were continued during the study. At 24 weeks, lung function in the tiotropium group had increased modestly (roughly 100 cc) from baseline FEV1. The interval at which 25% of patients had experienced severe exacerbations was significantly longer with tiotropium than with placebo (282 vs. 226 days). At 48 weeks, the number needed to treat with tiotropium to prevent 1 severe exacerbation was 15. Asthma symptom scores did not reach the predefined clinically significant endpoint. Adverse events were similar in both groups.

Comment: Overall, improvement in lung function and exacerbations was modest when tiotropium was added to therapy in patients who already were receiving high-dose ICS and LABAs. That said, we have little else to offer patients with severe asthma and fixed obstruction. Although tiotropium is not FDA-approved for asthma, I think it is a reasonable option for patients who continue to have exacerbations that require oral corticosteroids or hospitalizations despite maximal asthma therapy. In an accompanying editorial, the writer cautions against extending this finding to all asthmatics, because fixed obstruction might share features with COPD and thus be responsive to the anticholinergic effect. Also, she points out that tiotropium (especially in the fine-mist device used in this study versus the dry-powder device we have in the U.S.) might contribute to risk for death from cardiovascular causes and cautions against its use in patients with cardiovascular disease.

This is the ratio science suggests is ideal for our diet. This is about the same ratio that fish has. Grass-fed beef usually has less than 10% of its fat as saturated. If you are a pregnant or breastfeeding mom, the extra omega 3 from the grass-fed beef will provide incredible nutritional benefits for your child.

Grass-Fed Beef Enhances Your Health in More Ways Than One…

Compared to grain-fed cattle, grass-fed beef is:

  • Two to four times richer in heart healthy omega 3 fatty acids.

Eric Schlosser, who wrote Fast Food Nation, tells us that since 1993, half a million children in America have been made ill by the E. coli bug.

The USDA imposed rules reclassifying as “safe for human consumption” animal carcasses with cancers, tumors and open sores. Do you really want to eat beef like this?

I Have Been Eating Grass-Fed Beef Since February of 2001


I really enjoy the taste and the health benefits of grass-fed beef. I do prepare a good deal of ground beef for myself as that is easy for me to put in my salads. I have found that I can cook four pounds of the ground beef and there is only about six ounces of fat that I drain off.

But the amazing thing is that the fat is a completely different color than grain-fed beef. It is nearly clear and much thinner. This should not be surprising, but is an interesting physical confirmation of the huge difference there is in the fat between these two types of food.

The flavor, look, smell, and texture of grass-fed beef differs slightly from grain-fed beef. Many enjoy it immediately because it actually tastes like beef is supposed to taste. Others, accustomed to the blander taste of mass-produced beef, may be surprised by its fuller, richer flavor.

I have many patients who were raised in rural sections of Europe where this is the only type of beef they ate. When they came over to the US they were very disappointed with the flavor of our grain-fed beef. So it is all a matter of what one is used to.

Due to the lower fat content in grass-fed beef, you may want to experiment a little with various cooking methods to find what works best for you.

Real Grass-Fed Beef Can Be Difficult to Find

Much of the beef you find, even in health food stores, is from grain-fed cattle. Just because beef is labeled “organic” doesn’t mean it is grass-fed. The label must specify “grass-fed” to be truly grass-fed beef!

It may be true that the animals are given grains that are organic. However, organic grain-fed beef is only marginally better than commercial grocery store beef where the grains the animals are fed are not organic.

So don’t pay twice as much for organic grain-fed beef and continue to get inferior quality.

The best way to purchase grass-fed beef is to find a local rancher who is raising grass-fed cattle and then purchase the meat from him. This will save you the shipping.
